Is hospital transport reimbursable?

I have to attend a clinic for cataract surgery and prior to that I will have several outpatients appointments. I do not have transport or anyone who can take me and this is worrying me.

I am in the health service with Cpam and have top-up insurance and also a war disability pension. How can I obtain transport, including for post-surgery check-ups? I am 71. R.R.
