France issues new confinement form to include time
France has issued a new version of the form that must be filled in every time you leave the house during the confinement period, with a new space to write in the exact time of your trip.
The new form was put online yesterday evening (Tuesday March 24). It should be used from now on.
You can download the new form here.
It is also available as a Word Document or Text Document from the government website here.
What is different about the new form?
The new form has a space to write down the time that you leave the house, to respect the stricter restrictions put in place from Tuesday March 24.
These included imposing a time limit of one hour - once per day per person - on physical activities such as going for a walk or a run; walking the dog; or going out with children. (If you are out with your children they must be listed on your form.)
You must also remain within a radius of 1km of your home, and not mix with other people, except those you already live with.
The one-hour time limit does not apply to essential food shopping or necessary trips to care for vulnerable dependents.
The new rules also say that you may only leave your house for medical reasons if mandated by a doctor, or for any appointments or treatments that cannot be done remotely, or in any other way.

Can I still use the old form?
So far, yes. The interior minister has said that the previous form is still valid, but you must write the exact time of leaving your house on it manually.
It has not yet been confirmed how long the old form will be valid for, but it is still valid so far.
What if I don’t have a printer?
Exactly as with the old form, you are permitted to write the form out by hand on a blank piece of paper, including all the necessary information.
Do I have to copy the entire form out only to tick off just one reason?
No. If you are writing the form out by hand, you can simply write the one reason you are leaving the house, and do not have to write down every possible reason given on the original form only to tick off just one.
However, you must include all the introductory information too, including the title "Attestation de déplacement dérogatoire, en application de l'article 1er du décret du 23 mars 2020”, as well as your full name, title, date of birth, place of birth, address, today’s date, date and time the form was filled in, and your signature.
What if I need to go out for unavoidable work?
There is still a separate form to use if your reason for leaving home is professional. Download this here.
It is also available as a Word Document or Text Document from the government website here.
Can I use a phone or a digital version of the form?
No. It must be printed or written out on paper.
What happens if I don’t have a form or breach the rules?
Breaking the rules or being found without a form are punished by a fine of €135, rising to €375 if unpaid, and up to €1,500 in case of several repeated offences.
Police have the power to impose fines and even arrest repeat offenders.
